How To Get Approved For An Ohio Marijuana Card
If you are living with a qualifying condition, or you believe you have one of the conditions listed above, then you'll need to schedule an appointment with a certified medical marijuana doctor to obtain your Ohio marijuana card. At the appointment, you and the doctor will discuss your medical
condition(s) and treatment history. It is helpful for the doctor if you can bring any relevant medical records documenting your qualifying condition. By bringing your medical records to the
appointment, you significantly increase the likelihood of receiving a recommendation from the doctor.
Assuming the doctor is able to confirm your qualifying condition, you'll receive a signed recommendation following your appointment. The doctor's signed recommendation will
then be submitted to the Ohio State Board of Pharmacy. You submit a $50 registration fee to the Board of Pharmacy. They enter you into the patient registry. After your submission, you will receive
your Ohio medical marijuana card in the mail (or via email).

An Ohio Medical Marijuana Card is used to demonstrate that your name has been added to the Ohio Medical Marijuana Registry by a certified physician. With this card, you can:
1. Buy marijuana from dispensaries

Am I guaranteed to
get approved for medical marijuana after visiting your office?
We can’t guarantee approval for anyone, as the doctor is the only person that makes the determination whether or not a patient qualifies. To make it more likely that you receive approval from the
doctor, we recommend that you bring as much information as possible to your appointment regarding your medical condition and past treatments.
Cost of initial consultation and face to face medical evaluation with a
Resilience Health Systems Ohio Board Certified Medical Marijuana Physician.
Initial consultation is $50 non-refundable charge.
*Please bring a Ohio issued photo ID card
*Bring 1 or more medical record proof from you physician with 1 of the qualifying conditions (dated within the last 12 months).
*Complete all provided patient forms.
Second visit with the physician for your medical evaluation and consultation is $230. Veterans receive $30 off the second visit. Payment must be received before the appointment will be scheduled with the physician.
Upon approval by the physician for your qualifying condition the following will be issued and completed:
*Your picture will be taken for your Ohio Medical Marijuana Card
*Receive your first Medical Marijuana Defense prescription document.
*Your information uploaded in the State of Ohio system for regulatory and compliance with the Ohio Pharmacy Board.
